Description
Bird S. Coler Hospital was established in 1952. This accession consists of five patient admissions and discharges registers from predecessor hospitals to the Bird S. Coler Hospital. One side of the ledger is for admissions and lists the patient's name; age; religion; race; nativity; address; identification numbers; ward; and from where admitted. The other side of the page is for discharges and is completed in the approximate chronological sequence as the admissions portion. The information recorded consists of the date of admission; the patient's name; age; religion; the date of discharge and nature of discharge (whether deceased, sent home, or transferred to another facility); and an identification number. In a separate sequence of discharge registers, the information documented includes date of discharge; identification number; ward; patient's name; age; religion; nativity; date admitted; and nature of discharge. The separate discharge registers are probably for the Cancer Clinic. The registers are arranged chronologically.
